Attendance
Our on site & off site, private & group lessons are designed to be a weekly commitment. Three unexcused absences may result in a forfeit of your spot in our on site group lessons.
Cancelations
Lessons cancelled by the participant less than 24 hours in advance will be marked as attended and will not be credited toward the next season or off site lesson package. Cancelations on our behalf will result in a lesson credit. These may occur due to circumstances out of our control including but not limited to facility issues and weather conditions.
Health Screenings
Swimmers and observers will be required to complete this online health screening form before their lesson begins. This information will be used to complete contact tracing protocols as necessary.
Holistic Swim Co. reserves the right to refuse service to swimmers or observers exhibiting obvious symptoms of COVID-19 (heavy cough, persistent runny nose, fever) or other highly infectious illnesses.
Liability Waiver & Release
Unless otherwise noted, Holistic Swim participants hereby authorize any representative of Holistic Swim Co. to arrange to treat the Participant(s) for injury in any medical emergency during participation in aquatic activities under their supervision. Participants hereby waive, release and forever discharge and agree to indemnify and hold harmless Holistic Swim Co., any facility in which their program takes place, their shareholders, owners, members, directors, officers, agents and employees (the “Released Parties”) from any and all actions, suits, demands, claims, damages, attorney fees, and causes of action of any type or kind whatsoever arising out of or caused by the participant(s) in any aquatic activities at Holistic Swim Co. and/or it’s facilities.
In the event that the participant or their children should require medical care or treatment, they agree to be financially responsible for any costs incurred as a result of such treatment. The participant is aware and understands that they should carry their own health insurance.
Make Ups
If you would like to use a credit before the end of a season, you may request that a make up lesson be scheduled. However, requests can only be fulfilled if there is availability in an appropriate class.
Payments
Checks can be made payable to HOLISTIC SWIM CO., LLC. Electronic payments are also accepted. Payment for lessons is due upon receipt of an invoice. An estimate will be sent prior so that charges may be reviewed. Incomplete or missing payments will result in suspension. Ask us about our payment plans.
Refunds
Lessons purchased will NOT be refunded. Credits may be applied to a future semester.
Required Equipment
Swimmers are required to come to class prepared to swim: with athletic swimwear, goggles, a swim cap, a towel, and a face covering to wear on deck. Face coverings are not permitted while swimming due to drowning hazards.
Photo Release
Unless otherwise noted, Holistic Swim participants hereby authorize the use of photographs and video-recordings of participating children for advertisement purposes. They thereby grant and authorize Holistic Swim Co., LLC the right to take, edit, copy, publish, distribute and make use of any and all pictures or video taken of my child(ren) to be used for promotional materials and digital communications. This authorization shall continue indefinitely, unless otherwise revoked in writing. These materials shall become the property of Holistic Swim Co., LLC and will not be returned.
Supervision
Swimmers under the age of 15 must be accompanied to lessons by one parent, guardian, or caretaker who has been added to the approved on-deck observer list by emailing their proof of vaccination beforehand. Small children may accompany if needed. All on-deck observers over the age of two are required to wear a face covering over their nose and mouth.
